Application Letter addressed to the Vice Mayor requesting for accreditation;
Duly Accomplished Application Form for Accreditation;
Duly approved Board Resolution signifying intention for Accreditation for the purpose of representation in a Local Special Body;
Certificate of Registration or existing valid Certificate of Accreditation from any National Government Agency (NGA) or in the case of Indigenous People’s Organization (IPO) Certification issued by the National Commission on Indigenous Peoples (NCIP);
List of Current Officers;
For Civil Society Organizations (CSOs) that have been in operation for at least one (1) year, Minutes of Annual Meetings of the immediately preceding year as certified by the organization’s board secretary or Certification from the Board Secretary certifying the annual meeting’s conduct, including the date, location, attendees, and agenda;
For Civil Society Organizations (CSOs) that have been operation for at least one (1) year, Annual Accomplishment Report for the immediately preceding year;
For Civil Society Organizations (CSOs) that have been operation for at least one (1) year, Financial Statement, at the minimum, signed by the executive officers of the organization, also of the immediately preceding year, and indicating therein other information such as revenue, expenses, and the sources of funds.

Duly Accomplished People’s Council Membership Form;
Letter of Intent to be included as a member of the People’s Council;
Proof of existence and operation in the city for at least one (1) year prior to the date of application for registration;
Proof of activities held in pursuit of development objectives or organizational activities conducted;
Program of activities planned for the year following the date of application for membership;
Copies of its Constitution, By-Laws and/or Articles of Incorporation;
List of its officers and members of good standing and their respective addresses;
Financial Statement and Declaration of Assets and Liabilities, if applicable; and Certificate of Registration or Certificate of Accreditation, if applicable.

Civil Society Organizations (CSOs) Representing Vulnerable Groups are defined as groups who advocate and cater the needs of vulnerable sector such as Women, Children, Youth, Senior Citizen, People with Special Needs, Indigenous People, Poor and Transient Poor, Farmers, Fisher folks and the like, which are also comprises and/or constituted with the same people who are considered members of the said vulnerable groups.
